Residence Certificate
To Do: Contact the Colonel Visa on Line (Line ID: 0979242666) for a residence certificate. They will request a scanned image of your passport, visa confirmation letter, photo, and TM30 (see Setting Up House for information on TM30). Pick up your residence certificate from their office and pay 1,000 baht. Make a copy of your residence certificate.
To Bring: 1,000 baht cash per certificate
Medical Certificate
To Do: Make an appointment at Artima Medical Clinic (chiangmaidoctor.com). You will have a brief medical exam and pay 150 baht.
To Bring: Passport
Driver’s License
To Do: Go upstairs to window 27 at the Chiang Mai Provincial Land Transport Office to process your documents and receive a date to return and get a license
To Bring: Copy of passport, residence certificate, and medical certificate
Vaccines
To Do: Check the CDC’s website for a list of recommended vaccines. Walk in at Bangkok Hospital or the Chiang Mai Ram Check-Up Center and ask for the vaccines you want. Self-pay and reimburse through your insurance. Our team recommends getting the Japanese Encephalitis vaccine.
